Nelson Tasman Hospice provide specialist palliative care through an interdisciplinary team of doctors, nurses, therapists and trained volunteers, to patients and whānau living with life limiting illness.
Make a Referral
Health professionals can refer patients when the complexity of the illness is such that specialist palliative care is needed to achieve control of symptoms and where social, psychological and spiritual support would assist the patient and family/whānau. People may self refer, though we would request permission to confirm diagnosis with their GP or Specialist.
Our preferred option for referral is via our patient management system (PalCare)

Education
Our education team is committed to delivering a wide range of learning opportunities to support those who care for people in our community who have life-limiting conditions. They use a variety of interactive teaching methods focused on the needs of the participants. The aim is to provide and promote evidence-based knowledge and skills to meet the palliative care needs of people and their families/whānau in any setting.
