SERVICE SPOTLIGHT: In-Reach Palliative Care Service to Nelson Hospital
Our colleagues meet people every day who are surprised about the breadth of services that are provided by Nelson Tasman Hospice – we have grown so much over the years and the range of services that we offer have grown as well. Over the coming editions of our Hospice Matters newsletter we will focus on one service at a time and explain a little more in-depth about a service we provide. This month, we spoke with the team that provide an In-Reach palliative care service at Nelson Hospital.
Our team provides specialist palliative care advice, consultation, and education to support the hospital teams. Our team is comprised of Palliative Care Clinical Nurse Specialists, Nurse Practitioner and Palliative Medicine Specialist, supported by the Hospice Allied Health Team. Our specialist nurses are on-site Monday to Friday 8:30am-2:30pm. The hospital clinical teams can access phone support from the Hospice Medical Team 24 hours a day.
On discharge from hospital, some patients will be referred to the community Hospice service or the Hospice inpatient unit, others may not require the continued support of the Hospice service and will have their ongoing needs met by their primary care team.

It can be a stressful time, with issues such as a new life limiting diagnosis, an acute change of condition or when someone is actively dying. Together with the Hospice medical and allied health teams, we make a difference by supporting optimum symptom management and other psychological and social needs. For our rural communities, hospital admissions can be especially challenging, and we are able to smooth the transition with specialist knowledge, well planned discharges and communications.
