Workshop for Health Professionals: Supporting People to do their ACP

This series aims to provide education that will refresh or upskill all health care providers supporting persons with palliative care need. The education sessions provide learning which maintains and enhances participants palliative care knowledge, skills, and practice.

Topics will include:

  • Integrating advance care planning into clinical practice
  • How to Initiate advance care planning conversation using the shared goals of care framework
  • Understanding documentation of advanced directives
  • Documentation options
