Palliative Care Services
Specialist Medical Care
Provided by our palliative care specialist and four Hospice medical
officers at inpatient and outpatient clinics. Your own GP can continue
to care for you in the Hospice if you wish.
Home Nursing and Care Coordination
An experienced Hospice nurse will be your primary care coordinator.
She or he will visit you in your home to discuss your needs and discuss
the range of support services available. You and your family will be
supported by your designated care co-ordinator who will also provide
specialist advice and nursing care.
Hospice Unit
Sometimes people want to focus on the important things without worrying
about the daily routine. The Hospice unit offers a calm and caring
place for families to share quality time. Families are welcome to stay
overnight if they wish. Currently, there are four ensuite bedrooms
available if you need assistance with symptom control, relative relief
and end stage care. In 2008, we will be able to offer 10 ensuite rooms
and a family room.
Family Support Services
- Social Work. Our social worker can assist you to
access financial assistance as well as address personal matters that
may be of concern. This might include legal matters, advocacy to
services and facilitating family decision making.
- Counselling and spiritual care. Our counsellor is able to guide you through this difficult journey in a very human, practical and realistic way.
- Creative Living. Come along to this dynamic group to share a social and creative time.
- Library. We have a range of books, tapes and videos which may help you.
- Bereavement counselling and support.
Our team of volunteers have all experienced bereavement and can support
you on a one to one basis for up to 18 months. There is also a
bereavement support programme to help you work through the grief
process.
- Volunteer services. Our team of more than
300 volunteers enables us to offer more than 20 different support
activities which include sitting with patients to relieve caregivers,
helping patients compile ‘biographies’, delivery and installation of
electric beds to homes and many other services.
